It didn't count against the team scholarship totals but you still had your free ride to school. The downside was once you went on the medical scholarship you lost all your eligibility. That's my understanding of how it works. We (Auburn) had a TE that got his neck injured as a freshman, went on medical scholarship, and graduated this year.
